[00:57:33] Gynecomastia, by definition, is a growth of breast tissue in men.
[00:57:39] So all men have breast tissue, just like all men have nipples.
[00:57:42] And in most men that amount of breast tissue is small, but if the breast gland is large, then you would call it gynecomastia.
[00:57:50] So it's important to know that that's what the true definition of gynecomastia is, but some men also have other tissue in their breast area,
[00:57:58] like excess fatty tissue or extra skin.
[00:58:01] And so the gland and the skin and the fat, all those things can contribute to the way a man's chest looks from the breast area.
[00:58:08] area. Okay, so trying to distinguish between true gynecomastia and just the
[00:58:14] appearance of the chest secondary to body habitus. Is that a challenge? How do
[00:58:19] you distinguish between the two? So sometimes you can tell when you examine
[00:58:22] the patient, so you palpate the area and if there's any firm rubbery tissue, that's
[00:58:28] usually breast tissue. So you can kind of pinch the area, so there's ways you tell
[00:58:32] when you examine the patient, but you can also get imaging. So ultrasound is a
[00:58:36] really good way to see is there a gland and if so what is the size and is the
[00:58:40] size similar on both sides or are they different? And now when someone comes to
[00:58:44] see you or their family doctor gets referred to you is the concern pain is
[00:58:48] it cosmeticis or is it even something more serious like cancer? So most men
[00:58:53] who are seeing a plastic surgeon are bothered about the aesthetics. The first
[00:58:58] thing to know about gynecomastia like any other condition is you have to make
[00:59:02] sure it's not caused by any underlying medical problem and there's other
[00:59:05] medical problems that can cause it. For example, testicular cancer, certain
[00:59:09] abdominal type tumors, thyroid issues. You have to rule those things out before
[00:59:14] you decide what you're going to do from a surgical perspective. And that's the way we
[00:59:18] always do this. The history, the doctor takes a history, find out what's going on,
[00:59:22] they do a physical examination, try and determine if that's breast tissue or is
[00:59:27] it fat tissue, and then they do investigations, in this case like an
[00:59:31] ultrasound, and we come up with a diagnosis, right, and we find out is this
[00:59:34] the scatical mastia on its own, or is this secondary to an underlying condition that
[00:59:39] could be serious and needs to be treated, and then we get to treatment options.
[00:59:43] Before we get there, outside of the causes that are more serious like cancer, why did
[00:59:48] people get it?
[00:59:49] Is there something they ate, something that they did, why does it happen?
[00:59:53] In some people it's just bad luck, and it happens during growth during puberty.
[00:59:59] There are some other causes like testosterone use or anabolic steroids and marijuana can
[01:00:06] cause it as well.
[01:00:07] And there are other prescribed medications, for example, ones that control blood pressure
[01:00:11] that can cause kind of comastomy.
[01:00:13] No, I know a century ago, a conflict arose which would quickly become one of the most
[01:00:26] complex and controversial in the world. A conflict between two very different people
[01:00:32] for the same territory. To understand its origins, let's retrace the history of the
